Hospital bills group charges by 4-digit revenue codes. These are our own short descriptions of the common groups. Official revenue code definitions are maintained by the National Uniform Billing Committee.

The decoder flags a line when the charge is more than 2 times the Medicare amount, so you know what to ask about. A flag does not mean the charge is wrong. Medicare amounts: CMS 2026 Physician Fee Schedule and Clinical Laboratory Fee Schedule. CPT codes are from the American Medical Association. Plain-language names are our own. This is not legal or medical advice.
